Refer a patient

Referring patients onto Healthcare at Home is straightforward for both you and the patient. For more information, or to find out more how you can start referring, please contact us.

We only treat or deliver to patients who are referred to us by their consultant or GP, who retain clinical responsibility for the patient’s care throughout their treatment.

Registering patients

Clinicians must complete the appropriate patient registration form for the service they wish to refer onto. For services that require drug delivery then the relevant prescription template must be completed or own one provided. A patient will only be registered upon receipt of a full and correctly completed form.

Patient registrations should be sent a minimum of two weeks prior to delivery being required, although sometimes this varies with different services.

We are only able to contact patients to confirm their delivery if we have a valid prescription from their doctor. We advise therefore that the clinician should send the prescription at the same time as the registration form as sending them separately could cause a delay.

If purchase order numbers are required from the referring centre and the pharmacy department generates these, the clinician will need to follow the hospital’s internal process.

Delivery

In general, we will process all patient registrations and make contact with patients within five working days of receipt of a fully completed registration form, unless different terms are agreed within your Service Level Agreement. You can also contact us about a more urgent referral where we will do all we can to meet your needs and those of your patient.

We aim to give every patient as much choice as possible as to where, when and how they receive their treatment, within the clinical parameters of the service for which they have been referred.

Repeat prescriptions

When the first prescription is finished, Healthcare at Home will request a repeat from the contact named on the registration form. The first request will be five weeks prior to the next delivery date, prompted again at two weeks before and then again on the actual delivery date.
