How it works

Our services are designed to help to make your life as stress and hassle free as possible. Everyone is different, so we always try to ensure that our services meet your individual requirements.

Referral

Whether you need support after a spell in hospital, or whether it’s a case of delivering your medication to you and managing your prescriptions, you must be referred to us by a doctor (NHS or private).

What to expect

Once you have been referred to us by your doctor, we will contact you to welcome you to Healthcare at Home and arrange to provide our service at a time and place convenient to you. We will explain how the service works, and answer any questions that you might have. We will also send you some information about your service, including contact details for your Healthcare at Home team so that you can contact us whenever you need to.

Prescriptions, deliveries and visits

With your permission, we will request your prescriptions directly from your doctor whenever we need a new one, so you don’t have to remember to do this. We will also contact you each time that we need to arrange to deliver your medication, or arrange for one of our nurses or other clinical staff to visit you. Ongoing support

Whether you need to re-arrange a delivery or a visit, tell us that you’re moving house, or ask for advice about your treatment from a member of our clinical team, we are always pleased to hear from you and to help you however we can.
