Care Bureau
The Care Bureau is a single point of access for the patient and everyone involved in their care. Its multi-purpose IT and telehealth system manages everything from accepting patient referrals, to scheduling their home visits and offers round-the-clock telephone support and nursing triage, 365 days a year.
We provide:
- A system that supports the clinical service models by bringing together homecare and pharmaceutical delivery with freephone clinical advice and support, rapid response, electronic patient records and KPI monitoring.
- 24/7 clinical and administrative support including triage and care coordination, referrals, nurse and family liaison coordinators scheduling.
- A single point of access for patients and community-based health and social care professionals to all relevant and appropriate information about the patient using the service.
- Signposting and referral of patients to local statutory and voluntary services.
- Central database of patient notes, services, patient satisfaction reports and clinical outcome data enabling tracking of care and KPI reporting.

Our patients can ring up this service and speak to a qualified person who actually knows them, their case, and their relatives. When it’s two in the morning and a patient is feeling scared we are there to give them comfort and reassurance.
