Medication support

We dispense and deliver medication and ancillary items to patients or their carers at home, at work, or on holiday, providing them treatment at a time and place that suits them.

This service means the patient need not worry about seeing their consultant for repeat prescriptions or make regular trips to the hospital or pharmacy to pick up their medication. It aims to give the patient flexibility and freedom to simply get on with their lives.

Key service delivery elements include:

  • Prescription management with the referring centre to ensure the patient always has the right dose of medication available at the right time
  • Monitoring patient stock levels as a marker of adherence to the prescribed medication regime and to ensure efficient use of high value medications
  • Regular contact with patients to ensure medication is delivered as required and that patient contact details are kept up to date

The support that we provide patients includes:

  • Nurse visits to educate a patient and/or carer about the condition for which treatment has been prescribed: the correct use and storage of a medication, the management of side effects, what to do in the event of an adverse reaction etc.
  • Patient and/or carer training in the correct use of a medication-dispensing device, such as an injector pen or a portable infusion pump
  • Visits from our trained nurses to complete tests between treatment cycles and provide ongoing clinical support
  • Active remote support, such as a programme of telephone calls to educate, deliver messages and to remind a patient what medication to take and when.
